Methadone Maintenance Therapy

What Is Methadone Maintenance Therapy? The WHO-Recommended Treatment Approach for Opioid Use Disorder

Methadone is a long-acting opioid medication classified in Thailand as a Category 2 Narcotic—possessing legitimate medical value but strictly regulated and accessible only via a physician's prescription. In use since the 1950s, Methadone Maintenance Treatment (MMT) continues to be recommended by the World Health Organization (WHO) as one of the primary, evidence-based approaches for treating opioid use disorder.

How Methadone Works
Methadone acts on the same opioid receptors as other opioids, but with a significantly longer and more stable duration of action. When an optimal dose is tailored to the individual by a physician, methadone reduces drug cravings and alleviates withdrawal symptoms without producing the intoxication or euphoria associated with illicit opioids. This allows patients to resume daily activities, maintain employment, and care for their families normally.

Why Close Medical Supervision Is Essential
The appropriate dosage varies considerably from person to person based on the severity of dependence, overall health status, and individual response to the medication. A specialist evaluates, calculates, and gradually titrates the dosage while routinely monitoring patient progress to ensure maximum safety and prevent the risk of overdose.

Goals of Methadone Maintenance Therapy

  • Reduce opioid cravings and relieve withdrawal symptoms
  • Minimize high-risk behaviors, such as illicit drug use and intravenous injection
  • Enable patients to return to regular routines, work, and education
  • Serve as a bridge toward psychological therapy and long-term recovery
